About Us

Every family recognizes situations when their loved one needs a little help. These instances usually come up during the later years of life, while recovering from a major surgery or when adapting to illness or disability. This is where Community Home Health Care, Inc. can be your trusted ally.
Community Home Health Care, Inc. is a provider of home health care services. Home Health Care is helping seniors live independently in the comfort of their homes for as long as possible, considering the limits of their medical conditions. We also help adults cope with the stresses of disability through therapeutic services. Our staff includes nursing care and personal care experts to help those with illnesses as well.

Mission Statement

To provide patients with professional, comprehensive services which meet their individual needs in the comfortable environment of their own home. We value the life that you put in our hands.

Benefits of Home Care

  • No travel! We come to you! Services are administered in the comfort of your own home.
  • A specialized care plan will be designed for each patient.  One on One care ensures that services are properly administered under the supervision of your physician  or a registered nurse from our agency.
  • Services are available 24 hours a day, 7 days a week with schedules that can be adjusted according to your personal health needs.
  • Direct coordination with your physician on our response to the current care plan.  Adjustments to such care plan can be easily made upon request or as necessitated by your health conditions.
